Focal Segmental Glomerulosclerosis with Immunoglobulin A Nephropathy in a Patient with Unilateral Renal Agenesis

Abstract
The occurrence of focal segmental glomerulosclerosis(FSGS) on solitary kidney patient results from compensatory mechanism of hypertrophied remnant kidney. Although precise pathogenesis of solitary kidney is not well-known, the cases of Immunoglobulin A(IgA) nephropathy on single kidney were reported. But the concurrence of IgA nephropathy and focal segmental glomerulosclerosis on solitary kidney is not found. A 47-year-old woman was presented with dyspnea on exertion and both lower extremity pitting edema. Laboratory finding showed proteinuria and hematuria. Radiologic finding showed solitary kidney. To reveal the etiology, we did renal biopsy and confirmed focal segmental glomerulosclerosis superimposed IgA nephropathy. The patient was improved after medication, angiotensin converting enzyme inhibitor, calcium channel blocker, beta blocker and mycophenolate mofetil. We report a case of focal segmental glomerulosclerosis superimposed IgA nephropathy on solitary kidney confirmed on renal biopsy.
